Minor Consent to Participate form
This form is required if you are having your child participate in a martial activity in the SCA.
Medical Authorization for Minors
This form is required if you are having a child other than your own participate in a martial activity in the SCA. The parent must have the form filled outbefore the event.
Family Minor Waiver (for families with more than one child)
This form is required if you are having your child participate in a martial activity in the SCA.
Notarized Medical Authorization for Minors form
This form is required if you are having a child other than your own participate in a martial activity in the SCA. The parent must have the form filled out and notarized before the event.
